Your program baseline

Sample starting values—not global averages. Replace with one consistent annual cohort and a fixed 12-month persistence definition.

Gap-closing assumptions

Local planning assumptions, not measured PSP Connect effects. Percentage-point gains apply only to the supported fraction; recovery rates apply to the remaining gap.

Brixner 2019: discontinuation 51.4% vs 65.9%. Applying it to your gap is a local transfer assumption, not a predicted outcome. Not stacked with PDC or medical-cost effects.

Contacts, refill gaps & workload

Local planning assumptions, not measured PSP Connect effects. Percentage-point gains apply only to the supported fraction; recovery rates apply to the remaining gap.

Data & safety handoffs

Local planning assumptions, not measured PSP Connect effects. Percentage-point gains apply only to the supported fraction; recovery rates apply to the remaining gap.

Optional program economics

Provider operating budget only—not sponsor sales or guaranteed ROI. Negative values indicate added investment. Medical-cost comparators are excluded, and released hours are not counted twice. Cost placeholders start at zero.

Optional published-study comparator

US observational adalimumab evidence: not causal, not transferable to every therapy, and not a CareSync result. Applied only to the existing active supported cohort; no effect credited to new enrollment. PDC measures medication-covered days—not adherent patient counts.

Ranges are illustrative assumptions, not study confidence intervals. Custom edits affect the selected scenario only.

No published benchmark is used as a universal enrollment, consent, safety or retention target. Select therapy-specific definitions and validate assumptions with your clinical, pharmacovigilance and finance teams.

Gaps to investigate—not assumed universal rates

Enrollment leakage

Eligibility, consent and activation are different steps. Review incomplete enrollment and time to first meaningful support.

Continuity & refill access

Track persistence at a defined horizon, reasons for discontinuation and actionable refill gaps; distinguish access barriers from clinician-directed changes.

Uncompleted follow-up

Monitor completed contacts against planned contacts, outreach reach and educator queue age.

Fragmented reporting & safety ownership

Review required-field completeness, handoff ownership and acknowledgement time. Ownership coverage does not establish safety-reporting compliance.

How the models compound

  1. 1.

Eligible → consented → activated → persistent. Each downstream stage uses the preceding cohort; supported reach limits every improvement assumption.

  2. 2.

Contact delivery and refill assistance use activated patients. Persistence, PDC and refill support remain distinct measures; their effects are not multiplied or added as clinical gains.

  3. 3.

Workload compares baseline enrollment/contact administration with the larger modelled cohort, adds refill-rescue work, then applies supported-work efficiency. Growth can require extra staff time.

  4. 4.

Budget impact values only net administrative capacity and deducts added enrollment, digital support, fixed operating and implementation costs. Payer study comparisons remain outside program economics.

Integrated patient support · Brixner et al. · 2019

Mean PDC 64.8% versus 50.1% (+14.7 percentage points); 29.2% lower medical costs, excluding biologic/targeted drugs.

Sponsor-funded US observational adalimumab study; 2,268 matched patients. All-cause medical costs exclude biologic/targeted drugs; total cost including those drugs was not significantly lower (p=0.056). Associations, not causal effects. PDC is not the proportion of adherent patients. Supplied PSP report, pp. 4 & 7.

Discontinuation · same Brixner cohort · 2019

12-month discontinuation 51.4% vs 65.9%; approximately 22% relative reduction. The optional continuity comparison applies (65.9 − 51.4) / 65.9 to your local discontinuation gap.

Source-derived assumption, not a forecast. Same observational cohort as PDC and medical-cost findings: correlated endpoints, not independent additive effects. No universal PSP target is inferred.

Persistence & adherence · Srulovici et al. · 2018

Israel, 1,520 adalimumab initiators: median time to discontinuation 673 vs 574 days for PSP vs non-PSP participants.

Sponsor-funded retrospective Clalit cohort. Different health system; median days cannot be converted into a universal 12-month persistence gain. Context for continuity KPIs, not an applied multiplier.

Care-coach calls · Marshall et al. · 2018

Canadian observational cohort of 10,857 patients assessed care-coach calls, treatment-initiation abandonment, persistence and adherence.

Sponsor-funded adalimumab-specific evidence. Supports measuring enrollment-to-initiation and contact completion; no unverified numeric effect is inserted into the model.

Prescription abandonment · Brixner et al. · 2019

US commercial adalimumab study examined first-fill abandonment, distinct from later adherence or persistence.

Related sponsor/data source to the 2019 PDC study, with possible population overlap. Non-additive evidence for activation gaps—not an independent financial effect.
